AUTO PAY AGREEMENT

I understand that submitting this form authorizes Beltrami Electric Cooperative to enroll me in its Auto Pay program and to initiate variable entries to my checking or savings account through the financial institution listed above. I understand my enrollment remains in effect until I notify Beltrami Electric Cooperative to cancel my participation in the Auto Pay program.
